Learn when and how to complete a Badge Reassignment in order to report a dose for a wearer that differs from the original issued wearer.
When Can a Badge be Reassigned?
- Only UNUSED badges can be reassigned to new wearers. Why? If a dose appears on a report where multiple people have worn a single badge, there is no way to differentiate who received a dose, and how much. The only way to properly report dose for multiple wearers is for each wearer to have his/her own badge.
- Often times Badge Reassignments will be used for things like changes in personnel, site visitors, or corporate events.

Note: Changing the name on a badge does not reassign the badge.

How Do I Complete a Badge Reassignment?
- To get started click here to view the Badge Reassignment Form. Open it in a separate PDF viewer such as Microsoft PDF or Adobe Acrobat. Note that editing this document in a browser will not save changes.
- Complete the Badge Reassignment Form using the examples and instructions listed on the form as a guide. Where do I find my DTN?
- Email the completed form to reports@radetco.com to process the reassignment request.
Don't Forget! Completing a Badge Reassignment only changes the dose reported. To make changes to your future subscription service, login to MyRadCare to make edits. Need help? See our articles on how to add and remove wearers.
Where Do I Find the DTN?
1. On the badge: The DTN is the alpha-numeric code printed on the back of the badge.

2. In MyRadCare: Find the DTN by opening the Wearer dialogue box in MyRadCare and viewing the DTN of the badge issued for the desired wear period under "Badge History".
